Each American Medical Facilities Management (AMFM) care center has its' own personality,
features and communities that set them apart from the competition.
AMFM Nursing & Rehabilitation Centers are dedicated to the care of West
Virginia seniors and their families. Our long-term care services; guided
by our mission "to exemplify excellence in quality care to our customers,"
strives for the very best long-term care by our professional teams in each AMFM
care center.

Skilled Nursing
Each patient is unique in their health care needs, and
each patient's care plan is based on a complete assessment that addresses
the needs and goals of every patient.
AMFM care centers have a staff of Registered Nurses, Licensed Practical Nurses and Certified Nursing Assistants to meet the medical and personal needs of the patients.
Long-Term Care
Being close to family is important and AMFM understands those bonds
of family and friends; we encourage those bonds to remain strong. Each
center offers dozens of communities a close to home, convenient location
for their families' long-term care needs.
Along with medical services provided by a licensed physician, 24-hour nursing care and supervision services staffed by registered nurses, licensed practical nurses and certified nursing assistants, our facilities also provide:

Activities: Patients enjoy a quality of life full of activities that stimulate
and foster participation, socialization and The Certified Activity Professional
arranges a variety of individual and group activities for patients both
inside the facility, and trips out into the community.

Beauty & Barbershop Services: Look your best with our on-site
beauty and barbershop services. There
is a full range of services including haircuts,
perms, hair coloring, shaves and other related services.

Community Based Volunteer Programs: As a cornerstone of every community in which an AMFM care center resides,
a community based volunteer program is crucial to the high quality for
our patients.
Both young and old, individuals and groups, are encouraged to volunteer time and services at our care communities. These volunteers and volunteer groups bring aspects of the community into the center and enable the patients to enjoy qualities of life not under the expertise of the staff.
Dining Services
Dining and meals are an important part of each patient's day for socializing
and nutritional needs. The Certified Dietary Manager
supervises the menu to ensure each patient is meeting the dietary needs
outlined in the patient's initial assessment and plan, coordinate and
ensure all meals meet the dietary standards and requirements of every
resident.
In addition to meals, our dining halls provide the opportunity for socializing
and entertainment. Many of our Dining Halls have large screen televisions
playing favorite, classic movies, while others may have a piano for live
music. Each center infuses its own personality into each Dining Hall to
make a truly unique experience for patients, family and employees.
Environmental Services
Each Environmental Department excels at maintaining the appearance and
functioning of each AMFM care center. In addition to keeping the
care center campus beautiful for the enjoyment of the patients and their
families, the Environmental Department provides laundry and maintenance
services.
Family
and Resident Councils
Each AMFM care center is built upon strong community bonds and Family
and Resident Councils offer the opportunity to strengthen those bonds within
the AMFM care center.
Rehabilitation
You may need rehabilitation to regain strength and skills to return an
independent lifestyle after leaving the hospital. Whether regaining
your strength or recovering from surgery, our team of rehabilitation professionals
will help you reach the highest level of independence as prescribed by
a physician.

Physical Therapy: Trained and licensed physical therapists works
with patients to relieve pain, regain independence, strength, coordination,
range of motion and prevent disability or injury.

Occupational Therapy: OT evaluates, treats, and
consults with individuals whose abilities to cope with
the tasks of everyday living are threatened or impaired by physical illness
or injury, psychosocial disability, or developmental deficits.
- Speech Therapy: Speech Therapists help patients overcome
communication conditions such as aphasia, the loss of the ability to express
thoughts and understand language, swallowing difficulties and voice disorders.
- IV Therapy: Medications and liquid solutions that can be introduced into the bloodstream via a vein; these therapies can include nutrition, antibiotics or other drugs.
- Wound Care: A registered nurse as well as the doctor provides
care all wounds based on the individual patients'
needs.
Social Services
Each facility has a Licensed Social Worker (LSW) that works with families to ensure a smooth transition to post-acute care for patients and their families. The LSW will work with the patient and family on the admission process and educating the families on Medicaid, other payer sources and what to expect in an AMFM care center.
Also, the LSW is an integral part of the leadership team in working with patients and their families after the patient has been admitted and moved into their new home.
Transportation & Van Service
Each AMFM care center has a transportation van available for Activities’ use
for community outings and events.
